Job Description:
JOB SUMMARY
The Health Information Clerk manages patient health information
data by ensuring it's quality, accuracy, accessibility, and
security in the Electronic Health Record (EHR) through scanning,
filing, copying, assembling, and retrieving patient records.The
Health Information Clerk is responsible for the appropriate
disbursement of medical records per HIPAA guidelines. The Health
Information Clerk responsible for assuring records are routed to
the appropriate clinical staff per clinic policy in a timely and
accurate manner. Releases protected health information according to
federal, state, and departmental policies. Works under the
supervision of the department head.
ESSENTIAL FUNCTIONS
Is responsible for the integrity of the Electronic Health Record of
all patients.
Performs scanning and cataloging all documents into patient
Electronic Health Record (EHR).
Clinic and provider generated documents.
Outpatient and ER generated documents.
Pharmacy generated documents.
Patient generated documents and patient employer generated
documents.
Local, state, and federal agency generated documents.
Financial documents.
Other miscellaneous documents.
Prepares and disburses medical records per HIPAA guidelines.
Responds promptly and courteously to walk-in and phone requests
from patients for copies of health care information.
Responds verbally, or in writing, to requestors, as appropriate,
regarding requests which cannot be processed due to lack of
information, misinformation, missing or inadequate release of
information form, and delays due to incomplete medical records.
Ensures the appropriate creation and recording of fees charged,
billed, and collected for records and/or forms as requested.
Corrects duplicate medical record accounts.
Performs storage and/or destruction of hard copy of scanned items,
per HIPAA guidelines.
Is proficient in EHR applications related to position and
responsibilities.
Maintains confidentiality of all personnel and patient care and
relations information.
Follow hospital and departmental policies and procedures.
Must be free from governmental sanctions involving health care
and/or financial practices.
Complies with CCH Corporate Compliance Program including, but not
limited to, the Code of Conduct, laws and regulations, and hospital
policies and procedures.

JOB QUALIFICATIONS
Education
High school diploma or GED required.
Licensure
None.
Certifications required
See Cardiopulmonary Resuscitation Certification Policy and
Certifications/Education Requirements Policy.
Experience
Healthcare related field preferred
Microsoft Word and Excel preferred
Knowledge, Skills, and Abilities:
The ability to maintain continuing education requirements and all
certifications and licenses as required (e.g. ACLS, PALS, CPI,
CPR)
Ability to learn and utilize alphabetical and terminal digit filing
systems.
Must relate well with patients, families, hospital staff,
physicians, and public.
Ability to handle various projects simultaneously and efficiently
with attention to accuracy, prioritization, and patient
request.
Analytical, reasoning, and interpretive skills with a high level of
mental concentration and memory.
Computer Knowledge including the ability to enter material into
electronic record and a minimum of 60 accurate WPM. Ability to
